Placement and Transition

Placement in Canolfan Amanwy

All requests for special provision are considered on behalf of the Director at the Inclusion Panel on a weekly basis.  The County’s Inclusion Panel recommends placements in its specialist settings and this is the only route by which a child or young person can enter a specialist placement like Canolfan Amanwy.

Year 6 Transition

At Canolfan Amanwy, we work closely with local primary schools to ensure a smooth and supportive transition for pupils moving into secondary education. We understand that this can be a big change, and we aim to make the process as comfortable and personalised as possible.

 

 

How Transition Works

Our transition journey begins early, with staff attending Annual Reviews in the primary setting. Once the local authority confirms a placement at Canolfan Amanwy, we begin planning the transition in collaboration with the primary school, the pupil, and their family.

We aim to offer a minimum of five transition visits, tailored to meet the individual needs of each pupil. These visits typically include:

  • Day 1 – Morning visit: meet the teacher and explore the new environment

  • Day 2 – Afternoon visit

  • Day 3 – Full day with lunch, supported by a Teaching Assistant from the current school

  • Day 4 – Full day with lunch, supported by a Canolfan Amanwy TA

  • Day 5 – Full day with lunch, further supported by Canolfan Amanwy staff

Information Exchange Meeting

Before your child officially starts, we hold an Information Exchange Meeting involving parents, carers, and professionals to discuss your child’s individual needs and how best we can support them. These can take place as part of the IDP revue meeting.

You’ll have several opportunities to visit us alongside your child, helping to build confidence and familiarity. We also host an Open Evening during the summer term, giving families a chance to ask questions and share any important information ahead of September.

Leavers and Post-16 Pathways

Pupils can remain at Canolfan Amanwy until the end of Year 13 (age 18), with the possibility of an additional Year 14 if needed.

We are proud of the support we provide to help our pupils move confidently into post-16 or post-18 education. We have strong partnerships with Coleg Sir Gâr, where students can access Entry Level courses tailored to their needs.

As part of their final school year, pupils attend college link courses, fully funded by the school. These courses allow pupils to become familiar with the college setting, meet staff, and build confidence—ensuring they feel well-prepared and welcomed when they begin full-time college life.
